Irène vit seule dans une maison au bord de la mer non loin de Rome. Son père et son amant la croient étudiante. En réalité, sous le nom de code MIELE, elle aide clandestinement des personnes en phase terminale à mourir dignement en leur administrant un barbiturique puissant. Elle découvre cependant quʼil est en parfaite santé mais quʼil veut mettre fin à ses jours, ayant perdu goût à la vie.
